INSTITUTIONS THAT NEITHER REQUIRE STEP 1/ TOEFL, NOR HAVE ANY FEES:
These are the ones which are least in number because every one is looking for them, since they neither have any application fee nor have any tuition fee.
ALBANY MEDICAL COLLEGE (AMC), ALBANY, NEW YORK
Located in the capital of New York State, Albany, this institutions provides Free electives to foreign and international students. Academic and teaching depends upon the Consultant or Visiting physician with whom you will be associated during your rotation. It all depends upon your physician that how he uses you or to some extent that how you make yourself useful by your knowledge, interest and hard work. It provides electives in all the departments of the college.
KNOWN PROBLEMS:
1) The college does not provide accommodation of its own, you will have to find one for yourself.
2) The institute is not among the top most institutes of USA, so you may get visa rejection.
3) AMC also requires you, like all other institutes of New york to get a letter of eligibility from NY state education department. This is not a big problem and you can easily get that letter following instructions for that in the specific folder.
4) AMC also states in the policy that you need to send along with your application an English Language certificate from your college.
UNIVERSITY OF CONNECTICUT SCHOOL OF MEDICINE
They also don’t have any fee, and have a reputed hospital named, Hartford Hospital. You may need to be very quick to apply there, because they stop taking applications very early.
KNOWN PROBLEM
They state the policy of accepting the foreign students, but sometime they dont even respond on the emails.
INSTITUTIONS THAT DON’T HAVE ANY FEE BUT REQUIRE TOEFL FOR ELECTIVES
If you plan to take on toefl then it will open many institutes for you, likewise:
NATIONAL INSTITUTE OF HEALTH(NIH):
It does not have a medical school of its own, so its not mentioned in the website of extramural electives compendiu but it is a very highly reputed institute with programs associations with many top medical schools including John Hopkins etc. and has a wide variety of research programs.
Moreover the application procedure is simple, you have to send application electronically through their website. Then, you will have to send your transcript and LOR s by mail.
You will also have to give valid email address of one of your college faculty members and the email ID of Dean of Faculty/ Registrar for your reference. They will send the confirmation emails to those email IDs, and they must get the reply back to confirm your reference or your application will not process.
ADVANTAGE:
- It is widely known for its research projects and you can get one if you impress your consultant during your elective.

They don’t give their own housing but aid you by giving a list of available housings in the vicinity.
UNIVERSITY OF LOUISVILLE SCHOOL OF MEDICINE:
A very good institute and provides all the electives free without any cost.
Last year, they stated the policy on their website about the requirement of TOEFL.
But, recently few students got electives there without even taking the TOEFL. You can send a email to their elective co-ordinator stating that the medium of instruction in your institute is in English and that you will send a letter for that proof along with your application. So, you can take your chance and if luck goes you may get exempted from toefl.
Remember, these requirements or pre requisites are only to be checked by the elective coordinator and if somehow you succeed in convincing, you can get yourself exempted from these requirements. -
GOOD INSTITUTES THAT DON’T HAVE ANY FEES OR VERY LITTLE FEES, REQUIRES USMLE STEP 1 BUT CAN EXEMPT THIS REQUIREMENT
MOUNT SINAI SCHOOL OF MEDICINE:
This institute owns few of the top hospitals in USA. Along with Cornell, it is one of the best medical schools in NY state. They have some application fee around 250-300 dollars.
They can exempt you from USMLE Step 1 if you send them such a waiver from registrar or any faculty member requesting for your exemption from step 1.
They don’t require any sponsor.
Known Problem
They demand J1 visa for their electives , which is more difficult to get than B1/B2 visa. Moreover some people say you get a 2 year country restriction. However I am not sure about it. -
Anyways, you can overcome this problem by getting visa on the basis of letter of another university you are going for electives before or after this Mount Sinai elective. Once you are in USA, you can do electives in Mount Sinai.
HENRY FORD HOSPITAL, DETROIT
It has a number of hospitals in the Michigan state, which have good reputation.
They can also exempt you from USMLE step 1 on sending exemption letter and also provide you with housing on payment.
They need you to have a sponsor from any of their faculty members before you can apply there.

UNIVERSITY OF TEXAS AT DALLAS
They have some affiliation with UHS of Pakistan. You can also try your luck there. You can only apply in the departments of INTERNAL MEDICINE in this institute.
SOME OTHER INSTITUTES THAT DON’T REQUIRE USMLE STEP 1 BUT HAVE SOME FEES:
SUNY DOWNSTATE MEDICAL COLLEGE. NEW YORK:
They have 170 dollars application fees with 30 $ of NY letter. They provide accommodation to visiting students on payment.
KNOWN PROBLEMS
They send the acceptance letter quite late. For example, If you want to apply for march electives, your acceptance letter may come after January. So, your letter may be too late for visa.
They also need letter of eligibility from NY, State Education Department.
UNIVERSITY OF TEXAS, AT GALVESTON:
They have some application fees around 200 dollars.
But the problem is that they require you to come on F1 or student visa. F1 visa is usually of very limited time and the problem may arise if you also want to do other electives.
Again, you can get visa on the basis of some other university letter.
UNIVERSITY OF VERMONT, SCHOOL OF MEDICINE:
They only have a 100 dollars fee and have no particular requirement. They don’t require toefl from students who come from a medical school where medium of instruction is English.
KNOWN PROBLEM:
They don’t provide electives for the month of March and April.
TULANE UNIVERSITY SCHOOL OF MEDICINE:
They have a 300 dollars application fees plus 300 dollars tution fees.
KNOWN PROBLEM:
They have not been accepting too much students since the hurricane in their area, so your 300 dollars fees is at risk.
INSTITUTES THAT REQUIRE USMLE 1 AND HAVE VERY LITTLE OR NO FEES
If you have already taken USMLE step 1, then you just need to sit back and relax. Now, you have many many institutes open for you. Majority of the institutes have this requirement, so you have no shortage of options…….
Such institutes are:
CLEVELAND CLINIC is the best hospital in USA. It provides free electives.
MAYO CLINIC is the 2nd best, with 300 to 400 dollars application fees.
Other institutes are:
University of Cincinnati
University of Alabama
Albert Einstein college of medicine (only have 25 dollars fees)
Louisiana state university
Virginia Commonwealth University School of Medicine
Wayne State University and many more……. -
WELL-REPUTED INSTITUTES WHOSE BIGGEST REQUIREMENT IS THEIR FEES
If you are ready to pay their fees, then again you have no shortage of elective institutes.
YALE UNIVERSITY:
Their total fees is 3500 dollars with accommodation and malpractice and health insurance also included.
Weil Cornell Medical College
They have 2200 dollars tuition fees without accommodation with 100 dollars application fees.
HARVARD UNIVERSITY

There is no housing available nearby... if there is... it is extremely expensive... I stayed at a place about 1-2 miles away from NIH and you can get a bus about 100 m away that drops you right in front of NIH... it was good for me...
NIH visa requirement
I'm headed to NIH for an elective next month..but a problem with my visa cropped up...
I have a B1/B2 multiple entry visa & entered the US to do 3 months of electives and give my step 2 CS..at the port of entry, I requested the immigration officer to stamp B1 visa as NIH had specifically asked for it..he said that B1 visa can be given for only 90days and my stay was exceeding that so he has to stamp a B2 for me...when I insisted n showed him the NIH letter, he explained that since my visa is B1/B2, NIH should not have a problem accepting me...
Now 2 weeks before my rotation, I get an email from NIH that B2 stamp on I-94 will not be accepted. I explained what happened to them. They said I can still participate in the NIH CEP but my duties will be limited to observing!
Can u please tell me how much this will affect my elective? Please suggest what I should do..

University of toronto query for immigration medical examination
Hi umarbhai & others, I have got accepted for university of toronto in march for an elective in forensic psychiatry. I had to go to orthwestern university in february for another elective..Till then i wasn't getting any confirmation from university of toronto. But I still had hope, that i would get the elective, but i was running out of time in visa. So I applied well in advance for a visitor's visa for canada. After I applied and got the visa, I got acceptance from UoT. Now they write me an email that I will be requiring to undergo a immigration medical examination which will b required of me at the time of entry to canada. Now I already have the visa, so how should I go about it. Is there any1 who has done past electives at UoT or is gonna join me in march or would b goin in feb.
Q: Does the college ask you when you report them for electives, whether you gave the immigration medical examination or not ?
Q: If you enter Canada under reason that you want to meet your relatives, will it be ok if i am not able to give the exam.
Q: What is the exact way for applying this medical examination , in case u have the visas b4 handed bcoz I hav emailed the consulate, but they haven't replied back. Plus they don't answer on phone, they say to write an email. Actually the link you gave above is for UK and the elective co-ordinator said that "Generally these immigration medical examinations are required for those country that participate in Visa waiver program". When I asked, what about me as I come from country where vis has to be obtained ? She said consult your consulate.
The consulate people hav still not replied me. Can I give similar medical examination once I enter USA if I need to.
And another problem is that I know the doctors where I can get my medical examination done, but these doctors say that I need a letter from consulate stating that "Do medical examination of this guy"

I am telling you all this after talking to few of my friends who went to UofT. You cannot do electives on tourism visa, according to the best of my knowledge. You need to have a temporary resident visa for medical electives as far as I know and medical examination is required for all. Please visit the following two links

hey i had a query...all universities require that we complete our CORE clerkships in medicine, surgery, OB GYN, paeds, psychiatry...is there a minimum number of weeks of clinical rotations that we mustve completed to be eligible...cuz my trasnscript mentions the weeks that ive covered in each rotation....does anyone know that minimum requiremnet?

Mount sinai medical school only sponsor F 1 not J1 as mentioned above in the description and also if you are doing a elective in some other place on B1/B2 before joining mount sinai, they wouldn't allow you to change your status from B1/B2 to F1,
so if you are going to mount sinai, get your F1 visa stampled in your country and then leave so that you can change your visa from B1/B2 to F1(if you happen to have elective before mount sinai on B1/B2) by going to either Canada ( which require visa, so get that visa too in your country) or to maxico(which doesnt require visa from Indian Nationals)
mount sinai also dont allow you to do elective elsewhere if your F1 is sponsered by them.

If both are on F1 then it shouldnt be a problem as you have the option of changing your school on F1 without any fee. so contact the International Personnel office there.
As far as change of Status is concerned i dont know which one easy or not but i know that if you change from B1/B2 to F1 you have to pay 300 USD as Processing fee and have to fill a form (form 537 or 538 ,dont remember that well, but u should check the website)you have to make the request before quite a significant time and process do take quite a lot of time.So plan ahead.

I have done my elective there, the elective coordinator: Pat gasser is quiet strict, and she is juggling with a tone of applications all the time. I dont think she will agree to TOEFL sparing as you will be compared to other applicants with similar credentials, She is also strict abt malpratice insurance. Criminal background chk is NOT optional, you can obtain a Character Certificate from your district police or have your University Dean write a letter validating your flawless criminal background.
Attendings there are VERY FAMOUS, especially for Surgical specialties, You will find Fellows there from Duke, Brigham and other reputed places. So the environment is rich for makin future contacts ! my advise : if ur amog the fortunate to have taken your step 1 in time, then also do not be afraid of the pain to fulfil other requirements.

1- I think anyone wanting to get electives should try to apply to as many places as practically possible, grab the ones which they get first; suppose you get lucky enough to get selected at many places for the same months and same branches then find about those places and choose the one that you think would best suit you..
2- Every hospital/university has different requirement. It is advisable to start applying while you are in the prefinal year of graduation (4th i.e. final MBBS for Indian students).
3- If the university requires you to apply only if you have a Step-1 score and they are not waiving it for you, then the situation that you have described, will not arise. If they dont require Step-1 scores, its again not helpful to send your scores later on. You may argue that it will improve your CV, ok fine, so apply now and later email them your scores and and updated CV. Yes you can do that !!
4- Your dean should write, "as of now her expected date of grad is MMDDYYYY but if she takes a break to do elective rotations in the US her date of graduation will be extended by the duration of leave taken by her."
This will be a very important document. Get it certified by your dean separately. (btw its Viraat and not Virat :-) )

Most of my friends went for rotation in an IM subspecialty, like cardiology, gastro, oncology, hemat, endo. Some went for pediatrics. I went for surgery and neurosurgery, so basically there is no specific branch which I think is extremely difficult to get into for an elective rotation. I think its more of a matter of luck or rather hard luck if you do not get a branch of your choice. And more than what is easier/tougher to get into, your concern should be what to choose so that it later helps you. Try and go to somewhere where they take IMGs for residency coz once you get an elective somewhere you will sure be applying for residency there as your chances of getting there get high. :)

F1 visa
I am a final year medical student from Pakistan. I have got acceptance from Mount Sinai school of medicine for 1 month elective. I have been rejected for F1 visa 1 month back and I am applying again. students from my university who got approval from mount sinai had been facing this situatiion at the visa office that the VO is angry and irritated that i cant give F1 for 1 month elective, Mount sinai is sending applicants n not changing their policy. my frnd applied again, she had her intrview few days back and she had to face d same situation and was given a single port entry visa. but i am very worried as wott if she simply refuses dat mount sinai is not listening to us so v wont give u any visa. can any one help me as wott i can do in these circumstances?
