In order to help us continue this process of continuous professional enhancement, we need the help of our juniors, students we have been helping through this website. In case you are interested in reviewing content or compiling data for future posts of this website under my direct guidance/supervision, please let us know. hi .. i wana know if you have any idea about B2 visa extension. I have a multiple entry and got 3 months at port of entry. Is it advisable to apply for an extension or go back and come again. As i wana persue 2 more months of electives here in the US. Also lemme know if going to Canada is considered as a reason for new I-94.
what i have heard is extension is not advisable but could not find definite answer as to why so?

Thanks for your kind words... it is not essential but helps in many places... I m also a green card holder and even I gave TOEFL...

You dont HAVE to give TOEFL for electives but it increases your chances of acceptance, you can also get electives without it!

P.S: TOEFL result is valid only for 2 years!

Places like northwestern exempt you from TOEFL requirements if you are a citizen of english speaking countries like USA, UK, Australia, NZ etc.   They ask for your passport copy to be attached with ur application, to prove the same. But I am not sure about GC holder thing. Ask the office!

But on the contrary, TOEFL requirements are based on your medium of instruction and coomunication in home medical school. Which if you are from pakistan, would appear to US univs as non-English. So my advice is give TOEFL unconditionally. in the present era of fierce competition for electives toefl is a good hope bringer. All the places I got accepted are the ones which considered my toefl scores.

Yes it is.... MPH from a US university does help in getting residency in US... it is an extra degree that gives you an edge in knowing all the research protocol and stats.... I am not sure sure if MPH from a non-US university helps to that extent...

i have read people commenting about not to get excited about doind electives from big names  like yale,mayo, cornell or howard as they dont help you getting into residencies. they say it is good to have some usce from places where there are chances that they can offer you a spot in their residency program and not in these big places where they wont give you any spot orr call for ivs. plz reply

yes some ppl do suggest so..... as I see it is... u do electives at some good place like harvard, yale, hopkins and u have some chances of matcing there but it increases ur chances of matching in other places..... but if u do electives at some other place... u have good chances of matching in that one place but not much in other places as compared to students who went to harvard, yale, hopkins, mayo etc..... so at the end it is your personal decision...

hi i am a fmg from india i did my elective from mount sinai 2 months but i did it in 2 different fields one in dermapathology and second in pulmonary diseases medicine and i got 3 l.o.r 2 from derma and 1 from medicine .can you tell me that will derma l.o.r will help me to get in internal medicine residency that derma l.o.r states that i will be a good addition to any programme .or will derma l.o.r will go against my application

Bhai im a student of 9th semester MBBS from Pune. Iam willing to give USMLE exam ,but i have one problem which i want to share with you.

I heard from one of my seniors who are preparing for USMLE exam tht students who passed his MBBS with Grace marks in any of their subject they are not authorized to give USMLE exam.I had passed my ENT subject by grace marks,can u plz reply me and help me in solving my problem....

Could you please post the minimum weeks required for all the clinical clerkships done in medical school. I need this information for the transcript. ( The specimen transcript on this website contains minimum requirements pertaining to India only, I'm sure there must be some requirements by the US medical schools in particular). Thank you.

 I m sorry I m not aware of any.... though there are some requirements of minimum weeks per specialty in California for PTAL... but I am not aware of any US med school requirements... Generally Core Clerkships include 8 weeks IM, 8 weeks Surgery, 8 weeks Gynae-Obs, 4 weeks Paeds, 4 weeks Psychiatry... i think so... it should be like this...

the number of weeks required may be mentioned as a requirement for application to electives but if you do not fit in one such criterion then you can always email them and request to waive off that requirement giving certificate or documentation from your home school that in your school only that many weeks are required to complete the core clerkships. that should help

Hi Sir, As far as I know, not even USMLE is mandatory but also residency. Even the foreign consultants have to complete US residency training before practicing. An alternate route is to skip residency and apply for a direct fellowship... I don't think foreign consultants are allowed to practice without US training. Anyhow, your wide experience is impressive.

As Umar bhai mentioned, you need to pass USMLE and then get into and complete a residency training of OBGYN to get OBGYN board certification(at the end of residency u sit for this exam and then get license to practise OBGYN in USA) But even though I feel USA is the only country which has the least bias towards age in giving training( surgical branches do discourage older ppl coz they feel learning a new skill is easier in younger ages, as i have heard; but i am not sure). I have seen fellows of >40 yrs in NIH and they came here through fierce competition between their younger counterparts, so it is not impossible! But you have to weigh the risks/benefits. Another option of fellowships is also a good way. In order to get into fellowships you have to get ECFMG certified(or pass USMLE) and then apply. In some cases (I have no idea abt OBGYN), at the discretion of program director, the fellowship duration can be extended (to encompass more years at par with a OBGYN residency) and you may request the USA OBGYN board to allow you to sit for OBGYN Board exam or become board eligible(which u normally become after doing a OBGYN residency) and thus get a OBGYN board certification. But this is a very slim possibility. Personally I feel, given the highest rate of malpractice insurance for OBGYN docs in USA, it is difficult decision to start a new OBGYN career in USA. Thanks a lot for your kind words... it is totally upto u... every year many people dont even apply for electives... many don't apply in places with huge tuition fees.... many are not accepted even after applying... no sweat... moreover there are few big places with less fees.... e.g NIH is free of cost... for further details refer to the following link.

i talked to the dean about electives; and showed him the application forms and he was sceptical about everything, especially when he saw the part where he has to certify that the student is in the ifnal year of medical school.


im an intern now, and he says it can never be considered as final year. So is there any link available which can show my dean that

1) majority of students from india do electives during their intenrship>

 2) The us universities consider the internship of indian students as part of final year.
(I have this doubt. Do all american students do electives in the fields they like for as many months as they think they want after they complete 4 years of undergraduate training including core clerkships) (Dont they have anythin like internship?)

2. Final year of medical school is the last year before you get your degree. In India, as you get degree after internship so it is the final year of medical school whereas in US and Pakistan you get degree/ graduate before internship.

P.S: In US, the first year of residency training is called internship in most circumstances.

Every year many foreign graduates get residency and they don't have any electives and every year many foreign graduate with multiple electives don't get residency anywhere in US... so never consider electives to be a short-cut to residency or you cannot get residency without them... there r so many other factors... so like many other factors that boost your CV.... electives in one of them.. with USMLE scores always being the number one factor...alongwith your CV, personal statement, interviewing skills, visa status, research experience, job experience, time since graduation etc.

salam Umar bhai, iam new to your forum..study in final year SIMS Lahore.

i am focussing on the issue "not too late from graduation"....i have not yet completed my first read for step 1....have only read physiology till now....i have a 

relatively strong basics of mbbs(have scored above average in previous professionals)....i have read pharma and pathology solely from kaplan during the sessions

and have distinctions in written exams...iam willing to study from kaplan for the final prof as well(excluding surgery)...my final prof starts on 7th of Jan 2011...iam

confused on two issues and want your sincere advice:

1.should i go / prepare for step 2 OR step 1 from now onwards?....

2.should i try for electives now?.....

please guide me in a little detail to decide for my future.

try for electives, study from kaplan for final prof, then once you are done with prof, start for step 1 prep during or after electives, if u get electives try to give CS before coming but practice a lot for CS cases, atleast 3-4 times each case... I think it is the best plan for you!

my brother appeared for an interview at the US consulate for a tourist visa to do externship.He earlier went to the US and came back after giving his step 2 cs(he was given a 6 months single entry B1B2 visa)...now when he went for the visa again he was rejected and the reason they gave was that he should've applied for J1 visa,    he even showed them the acceptance letter from the hospital.The hospital where he got externship is ok with B1B2 visa(but it did not mention that in the letter).

he is going to appear again....what documents do you suggest he takes and what reasons should he give?

he will not be paid for externship by the hospital.

please answer.......replies will be much appreciated.

As everyone says, you are not late for USMLE if you are  applying for electives. Electives generally is the first step towards ur Residency application. Steps you can take later also but you can apply for electives only in that one year of your entire medical profession, so if you miss the bus you miss it (although later you can go for observerships etc.). So since you can apply for electives now, you should start working on it now. Keep reading posts and comments on this site and also other blogs/forums/communities. You will be at an advantage if you know the facts and figures beforehand.

It is a visit visa, so it wont be of much help... unless some university requires you to be on a visitor visa for electives, which is highly unlikely... most of them require B1, F1 or J1... I dont think you cannot use your current visa but at the same time it would help you in interview if you visited US and came back in time... it shows a good visa history...

Hi,

I am in Intern year in india. I wanted to ask if FMG portal externships are good enough to do. How are those in comparisons to the Uniersity Clerkships.

How does this work ? Do they give us Externships in reputed hospitals or to some private doctors. I am very confused.

I attended 2 months of electives in Yale and planning to apply for more. Should i pusue the path of University electives in US or FMG portal externships are equally gud.

hello everyone! i'm new on this, so please go easy on me..

my name is Lucas and i'm in the 3rd year of medicine school at USP, a brazilian university.

i've recently heard about "clinical electives" chatting with some friends. i was told that it was sort of a 4 to 8 weeks programme in which medical student were able to experience different enviorments in the field of Health caring.

However, dont know if my information is accurate, so i would like to know if someone could answer me  following questions:

1) who can apply for these clinical electives?

2) are international students eligible for this?(brazil?)

3) how much does it cost?

Welcome to this website! A clinical elective is a brief period of training in a US-based health care system. Since you are a medical student in Brazil, you would qualify as an international student. You are eligible to apply in any institution in US for electives provided you meet their requirements, such as the TOEFL exam and in an increasing number of institutions, USMLE Step 1 exam. The cost depends whether or not the institution has a tuition fees which can range anywhere from 0$ to 500$ to 3700$ per month.

Hi Dear Umer.. I am an IMG, went for a couple of electives for 4m and gave my cs in Philly, fate is that me and my other frnds who gave in philly failed. Worried if we can match with the fail in our profile. Dint yet give my steps and need your advise if we can match with good scores and some research experience,,

You can still go ahead.... just make sure you get done with others steps asap and most importantly, with good scores... CS failure cannot make you go unmatched with good scores and letters!

But I wanna get into a surgical or Ob/gyn residency.Mny ppl told me its herd to match into those, so Iwas thinking whether to go ahead and do a pg here and then try for residency after pg.What do you suggest Umer..I am really confused..please help me..

No doubt it is hard to match into surgery and ob/gyn with a failed CS... if you consider it a worth-taking risk, you can do pg training in your home country. Though I am not sure if your pg training helps you find a better residench spot.

Is this the same as required by University of Toronto? If yes, university of toronto accepts malpractice insurance only from their mentioned company. Different Canadian Universities have different malpractice insurance requirements; its better if you first consult someone from the place you are applying to!

can u tell me from where and which specific coarse should i do for hipaa training coz there are many universities and hospital which require hipaa training certificate for clinical electives...now there are many websites which offer certificate but i just want to make sure that i'm doing the right one...

hi how r u Umar bhai...I needed an advice regarding a major problem i am facing right now..My Form 186 has been rejected twice BECAUSE THEY SAY THAT seal doesnt match the one on ECFMG record ...I called ECFMG for help but they are not helping me in any way ...they told me to confirm wth the medical school whether they ve used the right seal or not which i did and they assured me that they ve used the right seal ..Infact i and my friends got it signed and stamped in front of our eyes and THEY ACCEPTED the other FORM 186 OF MY FRIENDS WID THE SAME SEAL ON IT ....now what should i do ?....im running out of time for my target date for step 1 exam so worried about this ??....Thanx..

Try sending a new form 186... make sure u have the correct seal on it this time... consult any of your friends/seniors who got their form accepted!

  hello.iam dr seema from pakistan and iam clear in fcps part1 in gyne obs in last exam in september.

i want to know somthing for mrcog part1.

iam from pakistan and clear in fcps part1 in gyne obs.it happend that after passing fcps part1 just fill form and register for mrcog part1.no need to give exam of mrcog part1.

but now i listend that offer is close now?

is it on or not?

plz tell me all about it?do i have to appear in exam or not?

if not than how to register myself for it?

i clear fcps part 1 in september21

Marrying a US citizen gets you a green card much earlier(in months) than a green card holder(in years). If a green card holder becomes a US citizen in the mean time, your case processing becomes faster(in months).


Marrying a US Citizen give immediate resident status (Green Card), so you do not need any visa to come US.

I am a 2nd year med student and i am planning to give my usmle step 1 around august 2012 but I have kaplan notes of 2010 so do you think i should get the latest edition of kaplan 2011 or 2010 notes are fine. Does it make any difference??? And also i have 2008 FA but for that i think i should get the 2011 FA , what do u think??? about kaplan and FA both.!!

It is better to use the latest edition but you can certainly use the older ones too; usually there is not much difference but even then, I always prefer the latest editions!

Dear Umar,

I need to know how to apply for a visa in india if both my parents work abroad . I was wondering specifically about the financial statement needed . 

Also, Im in final mbbs part 2 . My university exams are in nov/dec. And I want to go for an elective in the months of january/february/march, and also give step 2 CS then so that one big load of CS and USCE is off my head. 

I'd really appreciate your advice and opinion on the same i.e. what to do about the visa, and what you think about me applying now

I need you to clarify my doubts. my question is,

1. Is getting surgery electives is difficult?

2. If i do electives in surgery, will it increase my chances of getting surgery residentship in future??

3. If i do electives in someother dept, is there any possiblity to get surgery residentship?

4. also i heard that surviving in surgery residentship is a gr8 task, v can be terminated any time when v are not upto the mark, also it ll take 5-7 years to complete it..

are these true regarding surgery residentship?

    i really appreciate u for such a wonderful help in this website.i have completed my m.b.b.s from India this march.i heard that masters in public health is the best alternative for maintaining our visa status,at the same time an added advantage.is it hard preparing for steps doing masters simultaneously?what are the chances of getting into residency with and without mph degree?i heard its getting tough to get into residency even with double 99 scores,is it true?please help me out...thanks in advance ....

I'm a fmg, recently joined my intrnship but recently got my cs resukts and failed in ICE component:(.. I'm totally disappointed!!!!!!! I had great ambitions of getting into a university program.. How are my chances affected if i apply with cs passed in second attempt?? Please help!

P.S: I have three months of US clinical experience too with 3 LORS from Foreign docs.
